We collaborate with several hospice agencies for our patients who require hospice and palliative care. We work hand in hand with their case managers and nurses to provide patient care and follow up.
While in hospice care, a team of doctors, nurses, social workers, counselors, pastors, home health aides, and trained volunteers work together to meet the patient’s physical, emotional, and spiritual needs.
